Notice on the Launch of Electronic Health Declaration Form

Starting from 1 December, 2020, all foreign passengers who are flying from The Bahamas to China via a third country will be required to apply for the Electronic Health Declaration Form by logging on the website of http://hrhk.cs.mfa.gov.cn/H5/ via computer or smart phone.

Foreign passengers can fill in the information, declare the health status and upload the testing certificates online. Their application will then be examined and verified by the Chinese Embassy in The Bahamas before their departure from The Bahamas. Upon approval, they will receive the Electronic Health Declaration Form, which will be displayed in QR code with the "HDC" mark. Foreign passengers must present the electronic or printed code, and follow the procedures for inspection by relevant airlines during boarding. For test requirements please read the Embassy's notice on 2nd November 2020 carefully.

Chinese passengers (including the Hong Kong SAR, the Macao SAR and the Taiwan region) can still apply for the green health code with the "HS" mark via the WeChat Health Code app.
